Patient Management Program:
With every new specialty medication received at our pharmacy, the patient will be automatically enrolled into our Specialty Care Patient Management Program, which is designed on evidence-based standards of practice to optimize the patient’s outcome and have a significantly positive impact on the patient’s quality of life.
The program is designed to provide quality patient care while involving the patient, the clinical pharmacist as well as the prescribing physician.
The purpose of this program is to assess, monitor, and evaluate each patient’s adherence to their medication and treatment plan, as well as managing issues that might prevent the patient from being 100% adherent to their treatment regimen.
This program also allows the clinical pharmacist to identify and implement interventions to prevent disease or medication adverse events and manage side effects. Enrollment in our Specialty Care Patient Management Program is voluntary, automatic, and is provided at no cost to all patients receiving a specialty medication.
For questions on the program or questions on medications, please feel free to contact us 24 hours a day, 7 days a week. Patients have the right to decline participation, revoke consent or disenroll at any time simply by calling our pharmacy to opt out.
